1. Plan, provide, and delegate client-centered and coordinated care that promotes safe and high quality outcomes.
- I was able to plan, provide, and delegate care as I worked with nurse cole and fellow student nurse. Whenever the children came to the nurses office a problem I provide care by performing a focused assessment based on their complain, and proceed to givng the student the necessary medication. Through evidence-based practice she told each student to grab some tissue and sit with their head between their lap for 10-15 minutes, because that was how to stop a nose bleed, and within 10- 15 minutes the student’s noses stopped bleed and they were able to return back to class.
2. Engage in interdisciplinary communication effectively and employ client care technologies, information systems, and communication devices that support safe nursing practice.
- I observe nurse Cole engage in interdisciplinary communication by watching her call disciplinary officials to guide misbehavior students back to their classroom. She also was able to call the State office to transfer information regarding workers comp for an employee comp involved in a fall.
2.1 Integrate infromation technology to promote safe practice.
- I observed nurse Cole using information technology to promote safe practice, by making sure the students parents were fully aware of the care that their child received as well as giving out information to any child that came to the nurses office for a head related injury regarding what to do and what not to do.
2.2 Participate in intra and inter-professional partnerships, characterized by teamwork, collaboration, and communication.
- Nurse cole, the Del tech students and I were all able to perform collaboration and effective communication but taking turns
Communication is one of the basic survival skills of human and also a fundamental part of nursing. Effective communication would help to promote a positive nurse-client relationship which is crucial for the delivery of quality nursing care (Sheppard, 1993; McCabe 2003).
Communication includes verbal and written, as well as the ability to document data. One of the most difficult transitions, and a main stress factor, is the new graduates’ ability to communicate with the physicians. Having the ability to be a leader is a key to success. “To be an effective leader, a nurse must show collaborative teamwork, which is consistent with AACN core competencies. To fulfill this competency, new graduates must be able to delegate to others to promote patient safety and health” (Theisen & Sandau, 2013). Conflict resolution with team members and patients was a noted as a skill that new graduates lacked. “Several new graduates expressed concern that they were unprepared to deal with conflict appropriately and professionally” (Theisen & Sandau, 2013). Due to the lack of experience, new graduates are unfamiliar with the proper ways to deal with conflict successfully. Mentorship has been an effective way in improving new graduates confidence in dealing with conflict, because they were guided through real situations. Organization, prioritization, and time management was another identified problem. Patient-centered care refers to the view that patients and their family members are partners in developing a care plan. This stems from the belief that the patient is in control and that the care provided is rooted in respect that addresses the patient’s personal needs and values (Barnsteiner & Sherwood, 2012). Creating a partnership with a patient that allows them to grasp the goals and methods of their plan of care and includes them in the decision-making process can prevent errors from occurring. Successful teamwork and collaboration directly affects the efficacy of interdisciplinary communication. When effective communication is achieved within the healthcare team, this increases the amount of positive patient outcomes. Nurses and physicians must cooperate in order to attain a synergetic relationship. Their roles are integral to creating a safe environment for their patients. When all of these elements combine, the quality of care improves and collegial relationships are strengthened.
Nurses play an important role in achieving the competencies of interprofessional collaboration, quality improvement, and human flourishing. Successful interprofessional collaboration requires the willingness to step outside of one’s comfort zone and initiate interactions with other professionals. The Quality and Safety Education for Nurses (QSEN) Institute developed six core competencies: patient-centered care, teamwork and collaboration, evidence-based practice, quality improvement, safety, and informatics (Quality and Safety Education for Nurses Institute, 2017). At my facility, it is clearly evident that they have adopted these six core competencies to improve patient quality and safety. My facility created the Office of Patient Experience which supports care that is safe, of high quality and high value. Patient satisfaction is a top priority which is why our guiding principle is known as “Patients First”. Through teamwork and collaboration, we deliver care that is patient-centered by working together in multidisciplinary rounds on the inpatient units. Also, the nursing education department supports quality, safety and consistent nursing care through a database of policies and procedures developed using evidence-based research. Lastly, the nursing informatics department is working towards making our EPIC system more patient-centered. They are doing this by decreasing the redundancy in charting for the nursing staff and finding ways to improve processes which automate tasks. This in turn will reduce the time that the nursing staff spends with their computer and increase the time that the nursing staff can spend with their patients.
2. Explain how you and other nurses might work with the community and the population of interest to improve the health concern.
Communication is any form of expressing and receiving of messages between individuals. The importance of Communication in the nursing profession is to maintain high quality care for the patient but also maintain effective collaboration between professionals. Boykins, D (2014) states that the “registered nurse is expected to communicate in various formats and in all areas of practice”. Various formats include speaking to patients and coworkers as well as utilizing appropriate protocols and systems to effectively communicate regards to patient’s status.
When doing this, they become part of the interdisciplinary team, which consists of the nurses and all the other professionals providing care to the patient. It is an “approach in which two or more disciplines collaborate in the learning process with the goal of fostering interprofessional interactions that enhance the practice of each discipline.” (American Association of Colleges of Nursing, 2014, p. 1) Each healthcare member involved evaluates and develops a specific plan of care related to their field. When they communicate that plan, discuss thoughts and ideas, and continually reassess their plans with each other, a successful interdisciplinary team has been developed. Both teams are important to the patient and play huge roles in nursing.

When she finished a successful completion of 18 hours or more, she will be eligible for a nurse assistant certification. There were a total of three session, her groups was number 8 and she has an instructor named Fugate. It was assigned in clinical rotation depending on what date and times the students picked like Jennifer’s date, December 16. She excitedly explained the fact that she became more proactive and optimistic. As she learned the daily lives of the patients, she said, “There was no doubt that each and every patient expressed different verbally or by using their gestures to communicate; they must also be treated with love and care.” She really understood and acknowledge that helping others can be enjoyable. For instance, these practices like communication, observation, and documentation skills will useful to recover injured or disabled people. She knew some of the situations that the residents went through. While studying and interacting with patients, she proclaimed, “This was a necessity to get a basic grip on what you’re doing in the future and what you’re going to get into” Basically, this will get her a certified nursing assistant with college credits and opportunities. In this paper, I will go into further detail how providing patient centered care is challenging, how to overcome the challenges, how it relates to my chosen profession, and how this competency can impact delivery of care to patients.
