HIT 104: Basic CPT Coding
Lab – Week 4
Student Learning Outcomes
• Discuss guidelines & correct coding principles for subsections of the Surgery section in CPT.
• Abstract documentation from scenarios & health records in order to assign CPT/HCPCS codes.
AHIMA Competencies
:
• Validate assignment of diagnostic and procedural codes and groupings in accordance with official guidelines.
1. Refer to the guidelines for “Destruction” in the CPT Surgery Section of your CPT coding book. Per the guidelines, what is the definition of “destruction “? "Destruction" refers to the eradication of tissue by any method. This
includes, but is not limited to, electrosurgery, cryosurgery, laser surgery, and chemosurgery.
2. Refer to the guidelines for “Skin Replacement Surgery” in the Surgery Section of your CPT coding book. Per the guidelines, what types of materials are classified under “Skin substitute grafts?” According to the guidelines for "Skin Replacement Surgery" in the Surgery Section of the CPT coding book, materials classified under "Skin substitute grafts" include synthetic, biological, or biosynthetic materials. These products will form a sheet scaffolding for skin growth. Skin substitute graft codes are not to be reported for application of non-graft wound dressings (e.g., gel, powder, ointment, foam, liquid) or injected skin substitutes.
3. Refer to the guidelines for “Repair (Closure)” in the Surgery Section of your CPT coding book. Per the guidelines, how would repairs/closures for wounds on the same anatomical site or the same classification be coded? Per the guidelines for "Repair (Closure)" in the Surgery Section of the CPT coding book, repairs/closures for wounds on the same anatomical

site or the same classification should be coded by adding together the lengths of the repairs for the total repair of one wound.
4. Refer to the guidelines for “Destruction” in the Integumentary System subsection in the Surgery category of your CPT coding book. List one type
of lesion and one type of destruction method from guidelines. From the guidelines for "Destruction" in the Integumentary System subsection in the Surgery category of the CPT coding book, an example of
a type of lesion is "benign lesions." A type of destruction method is "cryotherapy."
5. Refer to the guidelines for Repair (Closure) in your CPT coding book. Per
the guidelines what types of materials are used in a wound closure? According to the guidelines for Repair (Closure) in the CPT coding book, materials used in a wound closure may include sutures, staples, tissue adhesives, and other devices.
6. Refer to code range 15773 – 15775 in your CPT coding book. Per the instructional notes, how many times can code 15773 be reported during the same session? The instructional notes for code range 15773 – 15775, code 15773 can be reported only once per session.
7. Refer to the guidelines for Breast procedures in your CPT coding book. Per the guidelines, if a puncture aspiration of a breast cyst is done, what codes would be used to report imaging guidance that is performed in conjunction with this procedure? If a puncture of a breast cyst is performed, in conjunction with imaging guidance, it would be reported using codes 19081-19086, depending on the imaging guidance modality used.
8.Refer to the guidelines for Breast repair/reconstruction. Per the guidelines, what is the goal of breast reconstruction? The goal of breast reconstruction, according to the guidelines for Breast Repair/Reconstruction, is to restore the appearance of the breast, either partially or completely, following mastectomy, injury, or deformity.

9. Refer to the guidelines for Flaps (Skin and/or Deep Tissues). Per the guidelines, what techniques or procedure are not included in codes 15570
– 15738? Techniques or procedures not included in codes 15570 – 15738 for Flaps (Skin and/or Deep Tissues) include adjacent tissue transfer, rotation flap, and other local flaps.
10. Refer to the guidelines for “Mastectomy Procedures” in the Integumentary System subsection in the Surgery category of your CPT coding book. Per the guidelines, what types of procedures are not included in the description for code 19303? According to the guidelines for "Mastectomy Procedures" in the Integumentary System subsection in the Surgery category, procedures involving reconstruction or revision of the breast are not included in the description for code 19303.
11. Refer to the general coding guidelines for the Musculoskeletal System.
Per the guidelines, what is the definition of “Open treatment”? In the general coding guidelines for the Musculoskeletal System, "Open treatment" is defined as any procedure in which the skin or mucous membrane is incised, or a previously closed wound is opened, to allow manipulation of the bones or joints.
12. Refer to CPT code 22206. Per the instructional note for this code what code(s) cannot be reported with code 22206? Per the instructional note for CPT code 22206, it states, “Do not report 22206 in conjunction with 22207” – lumbar.
13. Refer to the section on grafts and implants in the CPT Surgery Musculoskeletal System subsection. Per the guidelines for this section can
a code for the procurement of the graft be reported separately? When bone graft is used and is obtained in the same surgical field, it
is not reported separately.
14. Refer to CPT code 21315, per the instructional note for this code, how would a closed treatment of a nasal bone fracture without manipulation or
stabilization be reported?

Suppose your CFD code can handle nonelementary blocks. Combine as many blocks of Fig. as you can. The only restriction is that in any one block, the number of i-intervals and the number of j-intervals must be constants. Show that you can create a structured grid with only three nonelementary blocks. Renumber all the blocks and indicate how many i- and j-intervals are contained in each block. Add up all the cells and verify that the total number of cells does not change.

You are recently promoted as the chief traffic management officer of a major city in the country. Based on recent data, you learned that one of the leading causes of road accidents is over speeding. In your desire to minimize road accidents, you went out to observe how traffic management rules are implemented on the streets. Your attention was caught by a fast - moving car that seemed to be breaking the speed limit of 100 kilometers per hour(km/hr.), and so you followed it along the highway. After a short time, the driver of the car noticed you and started to slow down. Eventually, he stopped on the side of the road.
Supposed the speed of the car had been noted during this time and is modeled by the quadratic equation y = 3x² + 17x - 20, where y represents the speed of the car in km/hr. and x represents the number of minutes after you started following the car. How long did it take for the car to be under surveillance before it could finally stop on the side of the road? Did the driver…

Learning Goal:
To use the principle of linear impulse and momentum to relate a force on an object to the resulting velocity of the object at different times.
The equation of motion for a particle of mass m
can be written as
∑F=ma=mdvdt
By rearranging the terms and integrating, this equation becomes the principle of linear impulse and momentum:
∑∫t2t1Fdt=m∫v2v1dv=mv2−mv1
For problem-solving purposes, this principle is often rewritten as
mv1+∑∫t2t1Fdt=mv2
The integral ∫Fdt is called the linear impulse, I, and the vector mv is called the particle's linear momentum.
A tennis racket hits a tennis ball with a force of F=at−bt2, where a = 1300 N/ms , b = 300 N/ms2 , and t is the time (in milliseconds). The ball is in contact with the racket for 2.90 ms . If the tennis ball has a mass of 59.7 g , what is the resulting velocity of the ball, v, after the ball is hit by the racket?

Using signavio or any PBM tools and must Use pool and lane and object.
Model the following business process at a supplier. After a supplier notifies a retailer of the approval of a purchase order, the supplier can receive an order confirmation, an order change, or an order cancelation from the retailer. It may happen that no response is received at all. If no response is received after 48 h, or if an order cancelation is received, the supplier will cancel the order. If an order confirmation is received within 48 h, the supplier will process the order normally. If an order change is received within 48 h, the supplier will update the order and ask again the retailer for confirmation. The retailer is allowed to change an order at most three times. Afterwards, the supplier will automatically cancel the order.
