Mike is a vigorous man, age 40, who enjoys the outdoors. He has a small horse farm and regularly works his horses. He also is a part-time builder, performing small renovation jobs for his friends and neighbors. One day as he was drinking coffee with his neighbor, he suddenly bent over and cried out in pain. The pain was so intense he was soon on the floor writhing in agony. His neighbor called 911 and Mike was rushed to the hospital. Pain relief was accomplished with some very heavy doses of medication and the doctors set about trying to find out the source of his pain. Initially, Mike’s pain began with a dull, intermittent discomfort in his flank and in the lumbar region of his back. Then it intensified across his abdomen as he was drinking coffee with his friend.
