One of the essential aspects of planning care services is to have a holistic approach to planning and provision. This means recognising that all parts of person life will have an impact on their care needs and that you need to look beyond what you see when you meet them for the first time. I am currently an assistant manager i do work with customers every day so i do provide great customer service , knowing its all about the customers and to help them with any need because with out them we would not have our jobs. When looking at the services that had been previous delivered, these were not centred around the individual but the organisations who provided the service. People were expected to fit in and never given the opportunity or the choices in how it was delivered. In my workplace, each resident has a personalized care plan. We speak to individual about his life, his family, wishes and preferences. We ask residents what they want, how they want it, who should be supporting them, etc. Then we put support in place to help them to achieve their goals, we help individuals find positive aspects in their lifes. To keep positive changes in place we must keep re-evaluating the individual wishes to keep their care plan up to date. People respond to courtesy and feel comfortable when they know they have your respect. This helps them to be open and willing to ask questions when they don't understand something. By making people feel comfortable, you help them to be better listeners. Avoid words that require a dictionary when communicating cross-culturally. Pepper your communication with words that everyone understands.
I read the unit policy about emergency situations, accidents and incidents like fall. I also asked my preceptor about the forms that needs to be filled out when there are accidents and incidents like fall. When there is an incident of fall in the unit, the staff needs to act promptly to check the resident’s condition e.g. neurological vital signs, injuries sustained, any fractures, dislocations etc. The staff also needs to treat the resident’s wound if he sustained any injury from the fall. In addition, the staff should inform the resident’s family and fill out an incident/accident form and document it on the progress notes.

In this essay I will be exploring two different concepts and what they mean to individuals in different care settings. After this, I will discuss how these concepts can enable health practitioners to provide person-centred care for individuals in health and social care environments. Person-centred care in Australia refers to putting the patient first when it comes to their own health care needs. This is achieved by empowering patients to make decisions regarding their health once they have been provided with all the relevant information (Pulvirenti, McMillan, & Lawn, 2014). Person-centred care, also known as patient-centred care, provides patients with individualised, respectful health care. It includes ensuring patient dignity, readily available health information, in an appropriate language, involving patients in decision making, and identifying and respecting patients cultural, social and religious beliefs when providing care (ACSQHC, 2010).
A study into the nurse’s perspective regarding person-centred care by Ross et al. (2015) recognised that based on various studies by others there are many different interpretations of person centeredness. The study found that from the nurse’s perspective many things were essential to make up person-centred practice. The nurses identified staff’s attitudes, teamwork, empathy and recognising a person’s individuality as key components of person centeredness. Similarly, Scully (2015) recognised in order for high quality person centred care to be delivered leadership, delegation and communication amongst nurses is vital. McCormack and McCance (2017a) summarize these findings in their ‘Person Centred Framework’, which recognises in order for person-centred care to be successful it must involve everyone involved in the care and not just the person who is the patient.
