Onset and duration of concern Frequency and intensity of symptoms What the client wants to improve In response to the prompts above, begin typing here: Maria is a 26yo Hispanic female from Honduras who is seeking services today after police were called to her household for domestic violence concerns. Maria speaks very little English. She has an 8yo daughter, Sofia, who also will be needing services. Maria reports that she is from Honduras where she lived with her grandmother, mother, and nine brothers and sisters. At the age of 13yo, she began prostituting to help feed the rest of her family. She met her husband, Jaime, when she was 18yo and quickly became pregnant. Thereafter, she moved to the United States illegally and began working in a home in Teas. After she had her baby, she stopped working to care for Sofia who has had some medical concerns due to being born prematurely. Her husband, Jaime entered the United States illegally as well and has been working construction jobs to support the family. Maria reports that she has struggled with having more children which has contributed to her tumultuous relationship with Jaime. In addition, she admits to using drugs in the past, but denies current drug use due to her immigration status and having to care for Sofia. She also has reported that when Jaime gets angry that he hits and punches her. Recently, Maria shared that Jaime pulled a knife out on her after she caught him with another woman at the store. Since the abuse has started, Maria reports to having difficulty concentrating, flashbacks and thoughts that she was going to die. She refuses to engage further about the incident, but reports that he cut her. She gets very freighted and wants to crawl in a corner when she hears loud voices. In addition to the abuse, Maria’s 8yo daughter, Sofia, is possibly being sexually abused by male child that lives in the home. Maria doesn’t appear to be suspect any sexual abuse but reports that the male child comforts her at night and gives her lots of hugs and squeezes. Maria reports that she will not be kept in a system like the ones he was raised in Honduras to submit and receive whatever Jaime gives her. It appears that Maria is seeking counseling to get assistance with her abusive relationship. At
this time, the duration of the abuse and the counseling history is unknown. Family Structure Clients and their concerns are shaped by their family structure and stage of development. Maria was one of the primary providers in the household, which led her to drop out of since in the fifth grade to become a prostitute at the age of 13yo. Maria’s father has been absent since she was an infant. The significance of the relationships and the patterns she had with her family is unknown. Maria met her husband when she was 18yo and they both immigrated to the United States illegally. Maria and her husband share an 8yo daughter, Sofia. Sofia was born premature and has speech and other motor skill delays. Maria has struggled to have more children although Jaime would like to have more. Jaime blames Maria for not being able to have more children due to her past sexual history of prostitution. Maria’s family history of mental health, substance abuse, and violence is known at this time. Maria has openly admitted to using drugs in the past, but does not mention any family history. According to Erikson’s stages of development, Maria’s stage of developmental impact would be in stage 5, as this is the stage were adolescence between the ages of 12 and 18 explore their independence and develop a sense of self. In this stage, adolescents are tasked with identifying who they are and where they fit. Maria turns to prostitution at this age to find her identity by providing for her family. There are no known developmental challenges listed besides the delays of her daughter Sofia as mentioned above. Multicultural Considerations Clients and their concerns are shaped by a multitude of multicultural considerations and their intersectionality. Her religion is unknown. She speaks very little English and therefore may require interpreter services/resources. Maria appears to be heterosexual and is married to Jaime. Maria reports to growing up “very poor” in Honduras. Currently she lives with 3 other families in a condo, is unemployed, and depends on her husband’s income for support. Maria does not have a military upbringing. The cops were recently called to her home for a domestic violence issue between her and Jaime. Drugs were also found in the home, but Maria is adamant that it is not hers and that it possibly belongs to someone else in the home. Maria is in the United States illegally. There are several cultural considerations to consider in Maria’s case. Maria has mentioned Honduras culture as it pertains to the women being submissive to the men and taking whatever infliction the man ensues upon the woman. This is something that Maria has recognized as not being okay as she reports not wanting to partake in this cultural norm in Honduras. Another consideration is the language barrier between the counselor and the client. Maria speaks very little English which has can make it difficult for the counselor to provide thorough Services and interventions if Maria is not understanding or able to communicate Effectively. It will be the counselor’s duty to provide translation services to the client to ensure that Maria is understanding the resources and material that she is Providing. In addition, Maria is from Honduras which is one of the poorest Countries in Latin America. This is something as the counselor to keep in mind when discussing cultural differences between the counselor and client. Furthermore, it is imperative as the counselor to ensure that Maria feels supported and is given the resources that she needs no matter her immigration status. Maria appears to be very worried about her immigration status and therefore does not seek resources or services because of her fear of being deported. As the counselor, it will be imperative to reassure Maria to not allow her immigration status to deter from getting proper medical care and reaching out to available resources. DSM-5 Diagnosis
