March 24
th 2021 while conducting a night instrument flight rules cross-country flight the Mooney
M20B encountered weather that included clouds and introduced icing conditions. Shortly after a climb through the cloud, the engine began to run rough. Loss of power ensued, radar contact was
lost and the aircraft was located in the mountainous terrain. The money was on an IFR flight plan from Fort Stockton, Texas to Tucson, Arizona. the airplane was accumulating light rime ice at 8,000 ft mean sea level (msl).
He contacted Albuquerque Air Route Traffic Control and was instructed to climb to 14,000 ft msl. During the climb to 14,000 ft, the pilot reported climbing into clouds; the engine began to run rough and lost partial power. He applied carburetor heat and checked both magnetos. The pilot believed that he had lost both magnetos.
Safety Management Systems Process
Cross-country IFR flights already pose a lever risk, throw in a night cross-country flight during minor weather conditions and IFR only elevates the risk. Certain controls that could have been implemented would be to better prepare for a night flight. Exhaustion from flying at night could occur. Also, the crew could be better trained and icing conditions. Carburetor icing does not just occur in freezing conditions and the Pilot should have known that and taught that. I believe that fatigue could have led to his delayed act of turning on carb heat after climbing through the clouds.
