Module 02 Lab Activity - ICD-10-CM Coding Overview: There are 3 parts to this assignment. In parts 1 and 2, you will need to complete the table below. In part 3 you will use 3M encoder software and provide screenshots that show your work. Be sure you code to the highest level of specificity available based on the details provided in the diagnostic statements. Part 1: Coding using the Alphabetical Index Instructions: In the table below, read each Diagnostic Statement (in column 1), enter the correct Main Term (in column 2) and ICD-10-CM Code from the Alphabetic Index (in column 3). Steps for Identifying the Main Term and the Code using the Alphabetic Index For each diagnostic statement, 1. Identify the main term (see definition of the main term given in your code book). 2. Based on the main term you identified, use the Alphabetic Index found in the front of the ICD-10-CM code book to find the correct code. Do not use the Tabular List for this part. 3. Compare the details in the Alphabetical Index for the code to the details in the diagnostic statement. Ensure that it is the best match. Part 2: Coding Pathways using the Tabular List Instructions: Use the codes you identified in column 3 to find the complete ICD-10-CM Code from the Tabular List and enter it into column 4. Steps for Identifying the Code using the Tabular List For each diagnostic statement, 1. Look-up the code from column 3 and find that category in the tabular list. 2. Read any additional instructions associated with that category (e.g., includes, excludes, etc.) and compare it to the details provide in the diagnostic statement. 3. Based on that comparison, ensure you have reached the final code that best matches the diagnostic statement; if you have not, return to step 2. Page 1 of 6

Part 3: Use 3M Encoder to Determine Appropriate Codes Instructions: Use the 3M encoder to find the appropriate codes for the three (3) diagnostic statements below (A-C). After each statement, provide a screenshot showing that you were able to follow the encoder process to arrive at the correct code. Make sure that your screenshot shows the final code. Example Diagnostic Statement : Acute appendicitis Diagnostic Statement A: Pathologic fracture; left ankle Page 3 of 6
