CASE STUDY OF WILL 2 Description of Intervention The Historical Clinical Risk Management-20 assessment is an internationally utilized risk assessment for violence. Violence risk assessments allow for structured professional judgment decision-making to streamline the risk assessment process, allowing for the development of guidelines that connect best practices with empirical research. The HCR-20 was developed specifically for the assessment of general violence in forensic-psychiatric patients (Shepherd et al, 2017). Studies have shown there is sound predictive validity for both inpatient and post-discharge clients (Shepherd et al, 2017). A study that looks at the validity of assessment used for the Australian population found that the overall assessment had a strong positive relationship with the prediction of violent behavior if an individual had a history of such (Shepherd et al, 2017). Limitations of the study note that the clinical subscale was found to not be independently related to violence, with authors noting this could be due to the timing of assessment; patients assessed were at the point of discharge, where they were deemed stable (Shepherd et al, 2017). Another limitation noted was that researchers relied heavily on patient file data, possibly influencing the results (Shepherd et al, 2017). Also noted was the conservative outcome variable of “reconviction;” researchers suspect a stronger predictive validity could be garnered if variables relating to client arrest or charge history were included (Shepherd et al, 2017). Rationale I chose this particular risk assessment for Will due to his tumultuous relationship history and his developmental history. He is currently in a state of crisis, noting the need for immediate clinical assessment. Will has noted that in general, he has become a very angry individual,
CASE STUDY OF WILL 3 pushing others away with said anger and jealousy. Inconsistent love and care, as well as ridicule from his mother along with the confliction he feels in his multicultural identity, has led to the notion of emotional instability in the client; he feels insecure regarding his identity, being a biracial Hispanic/white individual who is also gay. He primarily identifies as Hispanic due to his upbringing, but notes he “looks white.” Due to his mother being harsh and unaccepting, he did not engage in any intimate relationships until the end of high school, citing fear of not being accepted. All of his past relationships had ended because of infidelity and him feeling “abandoned,” leading to him growing angry as time passed and begun to expect such outcomes. Explanation of Support Despite Will not having a history of active violence against others, he states he would like to go and confront his husband and affair partner after discovering the cheating, indicating a risk of potential violence. The first goal is to assist Will in de-escalating the current situation, as he is emotionally charged and in an active state of crisis. Once in a state where I can begin to run through the risk assessment, allowing for Will to take the time to recollect and answer as best as he can, I would try to dig for more information regarding history of violence, if any, then looking to relationship instability regarding the situation. Historically speaking, it seems Will is at high risk for relationship instability, and early maladjustment; these are due to Will’s overall romantic/intimate history and his tumultuous upbringing. Clinically speaking, Will holds many negative attitudes, as he finds his only current solution is to confront the affair. He has an accusatory history of infidelity marked by anger, fear, and jealousy towards his husband, it was never proven (until now) of any affair taking place. Regarding impulsivity, Will is suspected to have trouble with his anger management, which is associated with a higher risk of violence.
