They informed me that along with providing care for acute behavioral health treatment needs, they also function as a full-service medical facility, given the unique needs of the elderly population. Staff are given specialized training to serve and care for senior adults, allowing them to provide a safe and comfortable environment to help the patient and their family work on the necessary treatment goals. The facility provides structured therapy services in individual, group, and family settings to provide the most optimal environment for recovery/stabilization. This facility additionally offers all patients individual PT and OT evaluations within 48 hours of arrival. The faculty works closely with local mental health clinics, nursing homes, assisted living centers, etc to ensure the patient can access community-based care upon discharge. As noted in the conversation with staff regarding this program, this population varies greatly in its needs, as such, some ethical considerations need to be made before a patient is admitted. Some elderly patients may not be able to give informed consent to receive treatment; ACA code A.2.d notes that counselors need to balance the ethical consideration of including the client in decision-making, but also the legal rights and responsibilities of family

It must be known if an advanced directive is in place before such decisions can be made, as it dictates who can make what choices regarding the patient’s care in a psychiatric setting. This resource is best used if the individual is experiencing a mental health crisis. However, given her recent diagnosis and her age, it should be noted that this facility is better equipped to handle her unique medical needs alongside her mental health. The program offers medical support, medication education, support groups, and individual and group counseling. These unique supports can help her maintain her health with her cancer diagnosis while also recovering from a mental health crisis, should she succumb to one. Legally, to maintain continuity of care, releases of information would be necessary to inform me of any changes to medication or her treatment plan, as well as an overall discharge summary detailing her time at the facility. Additionally, as the client can make informed consent, at the moment an advanced directive is not a concern. An assessment is provided to prospective patients that review current issues and problems, observable changes in daily activities, and any medical or emotional issues that might be of concern. Often, these programs are a step down from inpatient treatment in order to ease the transition out of inpatient care into outpatient care. Not only does the program offer therapeutic groups and activities, but it also provides medical monitoring for those with health concerns. The team for the patient is comprised of a geriatric psychiatrist, an advanced practice nurse, a licensed social worker, and licensed professional counselors. The Emory Geriatric Clinic is located in Decatur, GA. Due to the varied needs of the elderly population, ACA code A.2.d should be considered, as not all patients of the population are able to make informed consent. Additionally, the counselor must consider if the client has a psychiatric advanced directive, especially in tangent with code A.2.d. GA code 37-11-4 outlines the parameters of such a document, including who can make decisions, what kind of decisions can be made, and when these decisions can be made. However, it should be noted this is a specialized advance directive for psychiatric care only. This particular program would be applicable if the client was transitioning out of an inpatient facility, but still needed a high level of support. Additionally, given her medical concerns, she must attend a program with easy access to medical services. As her medical care becomes more intensive, the provided transportation, meals, and medication management are beneficial to her as daily activities become more difficult. If the client were to experience a mental health-related hospitalization, this type of program would help her readjust to outpatient care and the additional health requirements she may have.
